Application Check List:   Photo Identification for all adults in the household.         Social Security cards for all members of the household.         Verification of household income for the past 60 days.         Paid receipts for the past 60 days.         All current utility bills.         Copy of current lease agreement or mortgage payment book.         Copy of income tax returns.         Medical verification (if medical conditions prevent you from working). Note: You may be requested to provide additional verification by your caseworker upon completion of your interview.
